    This includes Sexual Misconduct/Abuse Disclosure Release (Act 168) Forms sent by hiring entities or current/former employees for verification.  A HARD COPY must be submitted for completion.  Faxed or emailed forms will not be accepted.

    Beginning immediately, ALL applications submitted must have current clearances and COMPLETED Sexual Misconduct/Abuse Disclosure Release form(s) included for consideration of employment.  If you are in need of a form, please see the link below.
    As of December 22, 2014, all school entities and/or independent contractors are required to conduct an employment history review for any prospective employee prior to hiring, if the employee will be hired for a position where he/she will have direct contact with children.  Please provide the District with this Sexual Misconduct/Abuse Disclosure Release completed by any/all current employer(s) or former employer(s) in which is a school entity or where you were employed in a position that involved direct contact with children.
    The District reserves the right to close any Job Posting when a suitable applicant has been found.
