Sports Physical Packet 2026-2027

Fall sports physicals for the 2026–2027 school year will be held on Friday, May 29, at 10:00 a.m. in the high school gymnasium and locker rooms (grades 6th - 12th). Student-athletes are required to have a physical to participate in junior high and varsity sports. Any student-athlete will not be permitted to participate in practices, scrimmages, or games until a completed physical from a physician is on file.
The proper forms (CIPPE form) may be obtained from the high school office between 7:30 a.m. and 2:30 p.m., Monday through Thursday, in the high school principal’s office. (see below for form) The form must be completed prior to receiving the physical.  The cost of the physical is $20 cash.
